hollow temples and sunken cheeks

Hollow temples and sunken cheeks in Korea: what actually fills the shadow, and what does not

In Korea this is often called a “peanut-shaped face”: hollow temples above, hollow cheeks below, and the cheekbones look wider in between. The shadow can come from lost fat, thin skin, bone shape, or simply how light falls on your face, and each one is handled differently. A good consultation starts by working out which it is for you.

“My cheekbones look wider every year. Is that my bones or something else?”
“Will filler in my temples look natural, or puffy?”

Why it happens: more than one cause

01

Fat that has thinned with age or weight loss

The temples and the area under the cheekbones carry soft fat pads. As they thin, the bone above them stands out more, even though the bone itself has not changed.

How a doctor tellsThe doctor compares old photos with your face now and feels how much soft tissue sits over the bone.

02

Naturally prominent cheekbones

Some faces are simply built wider at the cheekbone. The hollows are the contrast, not a loss.

How a doctor tellsHollows have been there since your twenties and your weight has been stable.

03

Thin, lax skin

When skin loses firmness it drapes into dips rather than smoothing over them, which deepens the shadow.

How a doctor tellsPinching the skin shows thinness; the hollow changes noticeably when you lie down.

04

Lighting and angle

Overhead light and front-camera distortion exaggerate both cheekbones and hollows.

How a doctor tellsThe doctor looks at you in even light and from the side, not only in a selfie.

Your options, from lightest to most involved

A map for your consultation, not a prescription. The doctor decides which cause applies to you.

  1. Care and makeup

    Without any treatment

    • Contour and highlightA slightly lighter tone on the hollow and a soft shadow on the outer cheekbone changes the balance in seconds.
    • Hair with volume at the templesSide-swept or layered hair softens the temple dip; flat, pulled-back hair shows it most.
  2. Injections

    Injections

    • Hyaluronic acid filler →Fills the temple or cheek hollow on the day. Temples need a careful, conservative technique because of the blood vessels there.
    • Collagen stimulators →Juvelook Volume, Sculptra or Radiesse build volume gradually over months. Not reversible, so start low.
  3. Devices and lasers

  4. Surgery

    Surgery

    • Fat grafting →Your own fat moved to the temples and cheeks. Longer lasting, but some of the fat does not survive and a touch-up may be needed.
    • Cheekbone reductionBone surgery that narrows the cheekbones. Not in our current price list; we can still talk you through what to ask a surgeon.
  5. Doing nothing

    Doing nothing

    • Keep your structureA defined cheekbone is a feature, not a fault. If the hollow only bothers you in photos, styling may be all you need.
OptionWhen you see itHow long it lastsCan it be reversed?
HA fillerSame day, after swelling settlesMonths to about a year, varies by areaYes, it can be dissolved
Collagen stimulatorGradually over 1–3 monthsOften longer than HANo
Fat graftingAfter swelling, then some lossThe surviving fat is long lastingNot easily
Filling the hollow: how the main options differ

What we would avoid

  • Filling the cheekbones themselves when the problem is the hollow beside them. It makes the face wider.
  • A large volume of temple filler in one visit. Small amounts, checked a week later, look more natural.
  • Choosing a collagen stimulator for an event next week. It builds over months.
  • Booking cheekbone reduction on a short trip. Bone surgery needs a longer stay and follow-up.

Quick answers

How much is temple or cheek filler in Korea?

Korean HA fillers on our partner list run about $65–$105 per cc, imported brands about $180–$285 per cc. Ask how many cc the doctor plans for each side; temples often need less than people expect.

Filler or fat grafting?

Filler is quicker, adjustable and reversible. Fat grafting lasts longer but is surgery with more swelling, and not all the fat survives. On our list fat grafting runs about $715 for one area and $1,785 for the full face, before 10% VAT.

Can I shrink my cheekbones with injections?

No. Injections cannot make bone smaller. What they can do is soften the hollows around the cheekbone so it stands out less.

How long should I stay after temple filler?

Most people go about their day, with some swelling or bruising for a few days. Plan it early in a trip so the doctor can check you before you fly.
